OSH ACT of 1970 Section 1926.21(b)(2) the employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about July 31, 2018, at the Sealy Smith 854 site, employees pressure pumping the well were exposed to struck-by hazards when the swage separated from the well head flange.
