29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: (a) Mountain State Carbon, Follansbee, WV: On or about October 24, 2018 the employer had not recorded on the OSHA 300 log and 301 Incident Report a work related fatality that occurred on September 3, 2018 and that was recordable under 29 CFR 1904.5 in that an exposure to heat at the #8 coke battery operation contributed to the fatality.
