29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to protect employees from struck by and crushing hazards associated with employees maintaining equipment without written procedures for securing hazardous energy sources including but not limited to electrical, kinetic and gravity. The most recent occurrence was evident on November 7, 2018 during the investigation located at Mile Marker 368 Westbound Interstate 80 fifteen miles East of York, Nebraska, where the employer had employees repairing a disabled tractor/trailer without written specific procedures. While working on the equipment the employee was ran over by the rear tandem wheels of the trailer suffering fatal injuries.
