29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: a) On or about December 18, 2018, where a key was left in the lockout device for Pellet Mill #3. Employees were exposed to caught-in hazards when performing maintenance tasks such as the changing of dies.
