29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. On or about May 22, 2019 at the worksite located at 4806 Castlio Court, Saint Charles, Missouri, the employer failed to ensure that safe access was provided to employee(s) in the following instances: 1. An employee was observed installing siding on a garage from an approximate 10.5 foot high ladder jack scaffold with the brackets supporting the platform at the north and south ends of the scaffold turned outward. Access to and egress from the work platform required climbing over the bracket, which exposed the employee to a fall hazard. 2. An employee was observed accessing the roof of the garage to install a downspout on the side of the garage, using one of the support ladders for the ladder jacket scaffold. Access to and egress from the roof using the support ladder required climbing over the bracket, which exposed the employee to a fall hazard.
