29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. On June 4th, 2019 at the Cannon Place by Lennar subdivision, Cumming Georgia, employees were using ropes as anchorages that had been looped around a truss and back into itself. The system in use was not installed under the supervision of a qualified person.
