Moose Run Golf Course; on or about 18 August 2019, an employee operated a Polaris 6 X 6 Utility Vehicle, Model 204562, Serial number 4XARF68A694737528 along a designated golf course pathway without the use of the installed seatbelt. This condition exposed employees to a crushed by hazard.
