29 CFR 1910.421(d)(2): Planning of a diving operation did not include an assessment of the safety and health aspects of surface and underwater conditions: On or about November 12, 2019, a fatality of an Underwater Construction Corporation (UCC) in-water tender occurred during the initial set-up of a diving operation at the Exelon Nuclear LaSalle Station in Marsellies, IL. The employee was exposed to hazards due to the operation of a pump which may cause differential pressure (Delta P) changes along with other hazardous underwater conditions. The employer did not ensure diving operations included an adequate assessment that would include controls to prevent or correct such hazards that may arise during underwater activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
