29 CFR 1910.28(b)(1)(i)(C): (C) The employer did not ensure that personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems were used. Bronx, NY - Hellgate Substation 46: An employee was exposed to fall hazards of approximately 15 feet while standing on a transformer mounting bracket and/or it's components while attempting to change fuses on a transformer on or about December 5th, 2019. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
