29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) On or about January 13, 2020, at the Middle North (Loading Dock A), the employer failed to ensure the use of personal fall arrest systems for employees removing existing roof layers down to the structural metal deck, exposing employees to a fall hazard of 22 feet 10 inches.
