29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: RAM Construction Services of Michigan, Inc. at the worksite located at 226 Middle Ave. in Elyria, Ohio: On or about March 21, 2020, the employer failed to ensure that a floor hole cover was secured to protect employees from a ten-foot fall hazard. Plywood, used to cover a 3' x 7' floor hole, was not secured to prevent displacement.
