29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. a) BAYADA Home Health Care, Inc.- 115 West 30th Street, Suite 700, New York, NY 10001 The employer did not notify OSHA of a work-related fatality that occurred on or about May 07, 2020, as a result of exposure to COVID-19. On or about May 07, 2020, a Home Health Aide, died as a result of COVID-19. The employer learned of the fatality on or about May 07, 2020, but it did not report the fatality to OSHA until June 11, 2020.
