29 CFR 1926.1204(b): The employer did not identify and evaluate the hazards of the permit required confined spaces before employees entered them: On or about July 9, 2020, at or near the intersection of Mitchell St. and W. Barkley St. in Sour Lake, TX, where employees were exposed to toxic sewer gases and engulfment hazards after entering a sanitary sewer manhole without the hazards of the space evaluated prior to entry.
