1910.134(f)(2) The employer did not ensure that employees using a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter a) At the facility: On March 21, 2020 and continuing thereafter, where the use of respiratory protection including, but not limited to, 3M 1860, N95 Respirators, was required for environmental service employees, including housekeepers and porters, when entering the room of a COVID positive or patients under investigation (PUI), the employer did not ensure that annual fit testing was conducted. b) At the facility: On March 21, 2020 and continuing thereafter, when different makes and models of N95 respirators such as but not limited to N95 3M-1860, N95 3M-8210, SEKURA-N95 Particulate Respirator TC84a-7368, and 3M duckbill, were introduced into the work place, a fit test was not provided to nurses, certified nursing assistants(CNA), or environmental service employees to ensure an adequate fit of that N95 respirator. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
