29 CFR 1904.7(a): Basic requirement. The employer did not consider an injury or illness to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness. The employer did not consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness. a) Maybrook P-Denver OPCO, LLC doing business as The Gardens at Stevens � on or about May 15, 2021, the employer�s investigations were not timely completed to determine whether it was necessary to report any reportable injury or death on the employer�s OSHA 300 logs. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
