29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred: On or about January 11, 2021, at times prior thereto and at times thereafter, the employee did not record the following workplace injuries and illnesses: a. An employee tested positive for COVID-19 on January 11, 2021; b. An employee tested positive for COVID-19 on January 14, 2021; c. An employee tested positive for COVID-19 on January 14, 2021; d.
