29 CFR 1910.134(e)(2)(ii):The Employer did not utilize medical evaluation questionnaires which obtained the information requested by the questionnaire in Sections 1 and 2, Part A of Appendix C of 29 CFR 1910.134: a) Little Flower Manor Nursing Home located at 1201 Springfield Road Darby, PA 19023: The Employer did not utilize medical evaluation questionnaires which obtained all of the necessary information on employees health, in order to evaluate and make proper medical assessment on employees ability to use the respirator, before employees were fit tested or required to use respirators in the work place. The Questionnaires utilized by the employer provided limited information on employee's health. Condition occurred on or about January 13, 2021.
