At Interstate 95 Northbound, mile marker 256, Port Orange, FL: On September 8, 2021, at approximately 9:45 AM (EST) the employer had knowledge of an employee's work related injury and subsequent in-patient hospitalization, which the employer did not report to OSHA within 24-hours.
