29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness had occurred: On or about September 15, 2021, at times prior thereto, the employee did not record the following workplace injuries and illnesses: a. An employee tested positive for COVID-19 on August 18, 2021; b. An employee tested positive for COVID-19 on August 20, 2021; c. An employee tested positive for COVID-19 on August 27, 2021; and d. An employee passed away due to COVID-19 complications on September 17, 2021.
