29 CFR 1910.502(f)(1)(ii):The employer did not ensure a facemask is worn by each employee over the nose and mouth when indoors and when occupying a vehicle with other people for work purposes. The employer did not provide a sufficient number of facemasks to each employee to comply with this paragraph and must ensure that each employee changes them at least once per day, whenever they are soiled or damaged, and more frequently as necessary: (a) VA Oakland Clinic: On September 23, 2021, a Social worker and a Mail Clerk had a close interaction in the mail room where one of the employees did not wear a facemask and employees did not physically distance themselves. (b) VA Oakland Outpatient Clinic: Employees such as Social Workers were required to wear a surgical mask (Precept 15300) while performing their normal job duties at work. These employees were not provided sufficient number of surgical masks. Some employees wore the same mask for two to three days and did not store it in the office or at the worksite.
