29 CFR 1904.39(a)(1): Within eight (8) hours after the death of a worker as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about December 29, 2021, a worker suffered an injury that resulted in a fatality. The fatality was not reported within 8-hours to the local OSHA Area Office.
