OSH ACT of 1970 Section (5)(a)(1): The employer did not recognized hazards that were causing or likely to cause death or serious physical harm to employees in that a powered industrial truck operator was exposed to a caught-in-between hazard when operating the truck while not wearing a seatbelt: On or about October 31, 2022, at 5815 NW 18th Street Building 716F Miami, Florida 33126, an employee was exposed to a caught by hazard when operating a forklift while not wearing a seatbelt.
