Inc. at 5455 Brighton Blvd. Commerce City, CO : On or about December 24, 2022, and at times prior, employees were exposed to fire and explosion hazards in that dead legs were not identified or included in inspections and tests in Plant 1 #2 HDS/Rerun Unit #10.
