29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) 5800 Lake Bluff Rd, North Rose, NY, 14516: On or about 7/19/2023, the employer did not utilize a lock out tag out procedure to control hazardous energy during servicing activities (cleaning) on the Weiler Horizontal Mixer, Serial #76143, Model # M7225, causing an amputation of and employee's right hand.
