29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. On or about August 16, 2023, the path to ground on an extension cord was missing and not continuous, exposing the employee an electrical shock. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
