29 CFR 1910.29(b)(10): When the employer used chains or gates instead of a removable guardrail section at hoist areas, the chains or gates did not provide a level of safety equivalent to guardrails. On or about March 13, 2024, and at time prior thereto, in the warehouse/storage area; employees were exposed to a fall hazard in excess of 4-feet because the employer used single strand double loop chain(s) in the two hoisting areas but did not provide a level of safety equivalent to guardrails (i.e. Top-rail/Mid-rail).
