OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck by hazards during pressure testing a 60" diameter storm water pipe using a 60" high pressure Kevlar and Rubber pipe bypass plug: a) On or about March 20, 2024, inside the excavation, employees were exposed to struck-by hazards while pressure testing two 60-inch-high pressure Kevlar and Rubber pipe bypass plugs in a 60-inch diameter x 260-foot-long underground storm water pipe.
