Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)Parking Garage, Level 6, Bay 7-8, Line D-F - A 4" by 4" oak timber used to jack up a 12 foot by 60 foot precast concrete panel (double tee), did not have the capacity to support the maximum intended load of the concrete panel. The minimum jacking load required to separate the southwest stem of the panel (double tee) from its supporting corbel exceeded the allowable (safe) capacity of the wood jacking post, on or about 11/29/04. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Use 6" by 6" or higher grade 4" by 4" timber post with adequate blocking on top of the hydraulic jacks. 2) Follow Guidelines set forth for safe load capacity in the Standard Grading Rules for Northeastern Lumber, by Northeastern Lumber Manufactures Association, 2003, Rules for the Measurement & Inspection of Hardwood & Cypress, by National Hardwood Lumber Association, 2003 and National Design Specification for Wood Construction, by American Forest & Paper Association, 2001. b)Parking Garage, Level 6, Bay 7-8, Line D-F - Prior to the permanent welding of structural precast concrete member, temporary lateral bracing was not installed on the east side exterior column, the interior shearwall, and the double tee members, on or about 11/29/04.
