Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment free from recognized hazards that were causing, or likely to cause, death or serious physical harm in that employees performing maintenance on an overhead crane were exposed to a struck-by hazard from unsecured crane components. On or about April 10, 2006, at the Walgreens Distribution Center in Waxahachie, Texas, 2 maintenance employees were working on SRM crane 7. When the chain was pulled to put it back on the sprocket, the load handling device fell and struck an employee below it. The employee was under a suspended load that was not adequately secured to prevent it from falling. Among other methods, some feasible and acceptable methods to correct this hazard are: 1. Require the use of a block when servicing cranes to prevent the load handling device from falling. 2. Acquire and install brake upgrade. 3. Train all maintenance employees on the Automated Storage and Retrieval System and adjusting the SRM catch device.
