29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 9/7/2017 at 1565 Pleasant Hill Road, Duluth, GA and on or about 9/7/2017 at 19 Johnson Circle, Palmetto, GA: The employer failed develop and utilize specific LOTO written procedure for maintenance, service, troubleshooting, repairs, and removing debris on the Street Sweepers conveyor/elevator/hopper/brushes and other required equipment.
