29 CFR 1910.179(l)(2)(i)(b): All controllers shall be at the off position: a) West Crane; On or about March 24, 2018 and at times prior thereto, the employer exposed employees to struck-by and crushed-by hazards, in that the employer failed to ensure that all controllers to the trolley were in the off position while employees were working in and around the trolley man-basket.
