29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Throughout the facility: On or about September 27, 2018, and at times prior to, the employer did not ensure that emergency exit doors could be open without special tools or knowledge when an exit door was lock with a screw that required more than one operation to remove. In case of fire or other emergency requiring evacuation, employees were exposed to entrapment, burns, and smoke inhalation hazards.
