29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present which necessitated the use of personal protective equipment (PPE): a) On or about November 18, 2019, in the loading area of the Calumet Products facility, the employer did not perform a hazard assessment to determine personal protective equipment (PPE) needed to protect employees from overhead hazards when making pipe/tank modifications and for protection against exposure to chemical hazards when opening process lines.
