29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. (a) 3 Pavilion East, 4 & 5 Saint John, 5 Pavilion East & West, 9 Pavilion West and CCU/MICU: Employees working as Patient Care Technicians were required to wear N95 filtering facepiece respirators while providing direct patient care to COVID-19 positive patients. The employer did not ensure the employees received annual fit tests prior to March 2020. Violation observed, on or about 4/21/2020. (b) 3 Pavilion East, 4 & 5 Saint John, 5 Pavilion East & West, 9 Pavilion West and CCU/MICU: Employees working as Patient Care Technicians were required to wear N95 filtering facepiece respirators while providing direct patient care to COVID-19 positive patients The employer did not provide additional fit testing when different N95 respirator facepiece makes and models and sizes were used, on or after 3/25/2020.
