29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) Facility, 114-45 87th Ave, Briarwood, NY: On and after April 19, 2020, the employer required employees to wear tight-fitting filtering facepiece respirators, including, but not limited to, 3M 8200, 3M 8511, and Gerson-1730 N95 respirators, while providing care for or entering and cleaning rooms of patients suspected or confirmed to have COVID-19. The employer did not ensure the employees were provided a medical evaluation prior to initial use of the respirator. Abatement Certification and Documentation Required
