29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a. Between May 1, 2020 and May 26, 2020, employees who cared for suspected or confirmed COVID-19 residents at the Little Village Nursing and Rehabilitation Center in Chicago, IL were required to wear a N95 respirator. The employer failed to ensure a written respiratory program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was established and implemented.
