29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: Granite Nursing and Rehabilitation, LLC located at 3500 Century Drive, Granite City, Illinois: On or about 11/18/2020, the employer did not provide a medical evaluation to determine each employee's ability to use a respirator before requiring the use. The employer required employees to wear N95 and KN95 filtering facepiece respirators while performing food preparation work duties inside the facility.
