29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) Boiler house, Woodchip/bark-chip storage area; On or about December 7, 2020 and at times prior to, the employer allowed employees to work in the confined spaces of the woodchip/bark-chip storage area without first evaluating the space as a permit required space, exposing employees to caught-by (chain conveyor), entrapment, and engulfment hazards.
