29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Roof of 53 W Germantown Pike Norristown, PA- On or about July 6, 2021, employees were not protected from falling 24 feet through a roof hole opening to the concrete floor below. Employees were not tied off and other protective measures such as covers and guardrail systems were not utilized.
