29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Plant 2 - Edge Finishing, Tyler Machinery Co., Inc., dovetail machine, machine number WD212; On or about January 27, 2015, lockout or tagout devices were not applied to isolate all energy sources while machine operators performed servicing tasks including but not limited to pre-shift preventative maintenance check, setup, adjustment of machine parts, bit changes, and clean up.
