29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. Napoleon Spring Works, Inc. - Archbold, Ohio: On or about November 7, 2016, the employer did not ensure that the point of operation on the ARB Hinge Machine (Press #20) was guarded. Employees were exposed to crushing and amputation injury in that the machines press, which flanges metal tubes, was not adequately guarded.
