29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. Employee(s) were exposed to amputation hazards. On or about August 9, 2018, an employee engaged in maintenance activities handling ingredient scale #2 located in blending facility was exposed to amputation hazards in that specific procedural steps for placement, removal and transfer of lockout devices and the responsibility related to the pneumatic gate was not developed, documented and utilized.
