29 CFR 1910.421(d): Assessments made during planning of dive operations did not include all safety and health aspects specified in 29 CFR 1910.421(d)(1) through 29 CFR 1910.421(d)(9): 29 CFR 1926.605(e): Commercial diving operations were not in accordance with subpart T of part 1910, 1910.401-1910.441, in that 29 CFR 1910.421(d) was violated: On or about February 21, 2019, a SCUBA diver and attendant were retrieving a submerged irrigation pump approximately 50 feet deep at the bottom of a lake without the employer performing an assessment of; reserve gas supply and emergency procedures, exposing employees at the dive site to the hazards associated with SCUBA diving.
