29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment: (a) On or about 12/17/2019 - 7901 1st Avenue South, Birmingham, AL, employees engaged in activities such as, but not limited to, using hoses to flush out roof drains, were exposed to falls of up to approximately 60 feet to ground level when personal fall protection systems were not selected and required to be utilized by each affected employee.
