29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm) On or about February 13, 2020, at the New Sherman High School Project, employees were directed to perform masonry activities on a welded frame scaffold where the scaffold planks did not extend at least 6 inches beyond the end frame. The employees were exposed to an approximately 18 feet fall hazard.
