29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a) Boiler house, Woodchip/bark-chip storage area; On or about December 7, 2020 and at times prior to, the employer allowed employees to work in the confined spaces of the woodchip/bark-chip storage area without first posting signage that would identify certain areas as permit required confined spaces, exposing employees to caught-by (chain conveyor), entrapment, and engulfment hazards.
