29 CFR 1926.1412(d)(1): A visual inspection for apparent deficiencies of the equipment used was not conducted by a competent person prior to each shift the equipment was used or was not completed before or during that shift: a) On or about March 21, 2021, at the jobsite, a personnel platform used to hoist employees installing the arms of a power line monopole was not visually inspected for apparent deficiencies such as, but not limited to, missing pins or locked pins used to secure the platform to the crane.
