Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. 265 Newington Avenue (Foundry Area): On or about January 17, 2024, silica and aluminum oxide powder utilized by the employer to create ceramic shells for casting aluminum parts left a residual film present throughout the facility creating a slip-fall hazard for employees. 1) Foundry Area (Knock-Off Area): Knock-Off Area was coated in dust and crumbled ceramic shells from the air-chisel booth where employees remove the shells from the newly-formed aluminum parts. 2) Shell Room: Silica slurry from the dipping action of the Investment Mold-Making process covered the majority of the Shell Room floor creating a slip-fall hazard to employees entering the room.
