29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment. a) On or about February 20, 2024, the employer failed to ensure that each of the two employees involved in the service and/or maintenance task of line breaking on Asphalt Tank 150, each affixed a personal lock onto a group lockout device. Only one employee used a lockout device to lockout out Asphalt Tank 150, thereby the employees were exposed to hazards associated with the unintentional release of hot asphalt from the equipment.
