29 CFR 1910.410(c)(1): The employer or an employee designated by the employer was not at the dive location in charge of all aspects of the diving operation affecting the safety and health of dive team members: 29 CFR 1926.605(e): Commercial diving operations were not in accordance with subpart T of part 1910, 1910.401-1910.441, in that 29 CFR 1910.410(c)(1) was violated: On or about February 21, 2019, the employer did not ensure that a designated person was at the dive location and in charge of all aspects of the dive operations, exposing employees to the hazards associated with SCUBA diving in a fresh water lake.
